Pathway 3 is the 3rd pathway of the National Home First Initiative.
Pathway 3 was successfully piloted in West Kent in 2017and moving forward a new team has being created to manage patients who have complex discharge planning needs.
This group of patients no longer need to be in the acute hospital environment but require on going assessment to identify long term discharge planning needs this will be supported by Social Services and Continuing Health Care.
We commission beds in local care homes to provide a safe environment in order to continue the assessments for patients who are unable to return to their usual place of residence.
The team is multidisciplinary and multiagency and works in collaboration with both our health and social care colleagues in the acute hospital and the community setting.
This position is an essential central role within the team, expert knowledge of microsoft systems, day to day management of complex spread sheets and data systems, extraction of data to formulate reports, diary management and team co-ordination. Excellent telephone manner and empathetic approach when receiving calls from patients, relatives and carers is essential.
You would be working as part of a wider team in a very busy office environment but you will need to be self-directed, motivated, dynamic and an excellent communicator.

Successful applicants are required to have the qualification level specified in the person specification in order to carry out the job, however, we do welcome applicants from individuals wishing to further their knowledge and skills and to gain this recognised qualification, which we will support. So if you do not have the essential qualification for the role, this may not be an automatic barrier to you being offered an interview.
If you are successful at interview but do not have the essential qualification for the role, we will employ you as trainee and will support you to gain this qualification. You will develop new skills and knowledge whilst working with an established team, gaining valuable work experience and continuing to get paid. Once you receive the qualification you will automatically move into the position advertised.
