This is an exciting opportunity to work with patients to improve their health and wellbeing. As a Health Coach, you will join our expanding Social Prescribing team, working with a Primary Care Network in Dartford & Gravesham to deliver quality care and services to local people.

You should be a great communicator, able to quickly build rapport with patients and support them to take an active role in their health care. You will co-develop a personalised plan of care, helping people see how they can make changes and achieve objectives.

You’ll deliver group and one-to-one sessions on healthy eating, physical activity, and other health and wellbeing topics; providing advice and support for both patients and colleagues.

Working collaboratively with the multi-disciplinary team, you will help achieve the wider strategic objectives of the PCN to strengthen, develop and support local general practice.

Applicants must have relevant personal or professional experience, and of working with vulnerable adults in the social care, health, education or voluntary and community sector. Experience of working in a health environment, GP Surgery, Health Centre or Hospital would be helpful.

You must be able to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. You will work in surgeries and the local community so access to your own car is essential.
