The role:
- We are looking to recruit Band 4-equivalent healthcare assistant roles for a Severe mental health & wellbeing service we're running in partnership with Kent and Medway CCG.
- You will work with patients within primary care, providing comprehensive and systematic annual health assessments for those on the Severe and Enduring Mental Illness (SMI) Register, as well as a holistic review of their wellbeing.
- You will work alongside GP surgeries (with support from your manager), carrying out physical health checks and general health & wellbeing assessments.
- The Severe mental health & wellbeing service will operate as part of Live Well Kent - a countywide network of mental health organisations. This will make it easier for your patients to access other types of community-based support - in areas such as employment, housing, or social isolation - if they need it.
- It's important you're able to work autonomously with patients, manage your own caseload, and adopt a flexible and positive approach whilst having the ability to deal with competing priorities.
This role requires:
- Experience of working with vulnerable people
- Strong collaborative working skills
- The ability to work both as part of a team and independently
- A good awareness of safeguarding and risk management
- The ability to motivate and influence others
- A driving licence and use of a car (subject to the requirements of the Disability Discrimination Act).
