Social Enterprise Kent is expanding its social prescribing team and we are looking for a new team member to help support people in the Thanet district.

The Social Prescriber will use a holistic person-centred approach to improve or maintain the health, wellbeing and independence of older and vulnerable adults with complex needs. They will enable people and their families and carers to make best and timely use of the full range of available support and help across public, voluntary and community services. They will address social, economic and environmental issues, as well as health and care needs. They will work over a defined time period to enable people to achieve their personal goals, address immediate concerns, and ensure ongoing support is in place.

The Social Prescriber will work with individuals, to identify and build on their abilities, priorities, and existing personal and community networks. They will offer different levels of support according to personal goals, level of need and context, but the majority of cases will involve pro-actively supporting individuals to develop and implement a personal action plan in their own home. The Social Prescriber will improve the coordination and quality of interventions to improve health and well-being outcomes for individuals, their carers and families. They will offer a consistent supportive presence, avoid duplication of service provision by other agencies and ensure that care and support is personalised and timely. They will provide referrals if required, sign-posting, direct services. With an in-depth knowledge of local services and opportunities, they will build networks and relationships with local communities and services, highlighting any gaps in provision. A key part of the role is to link clients into existing services such a financial services, wellbeing services and more.

Social Enterprise Kent has established itself over the recent past as one of the main social enterprise bodies in Kent, giving advice to many charities and budding enterprises, and running training and employment services. We also manage the East Kent social prescribing service.

We welcome applications from people of all ages and backgrounds, and those who have personal experience of some of the issues we are trying to address in Social Enterprise Kent would be especially welcomed to apply.

This is a 12 month employment contract.
