Job Description Job title: Severe Mental Illness (SMI) Health and Wellbeing Worker
Project/Base: Gravesham or Swale (the role/s cover the former CCG areas of Dartford, Gravesham & Swanley and Swale based on local need)
Hours: 37.5 hours a week (some flexibility required)
Salary: £22,648 rising to £25,274 after 5 years’ service
Contract: Fixed term until 31st March 2023
Overall Purpose of SMI Health & Wellbeing Service provides the NHS England defined SMI physical health checks to people living with SMI and who are supported locally through primary care services. The companies approach to completion of the checks is to involve care and support planning around both health and wellbeing needs.
The service will provide one-to-one advice and guidance to people aged 18+ whose long-term mental health conditions places them at a greater risk of developing poor health outcomes. The service will offer a holistic needs assessment, referring and linking people to support from the wider Live Well Kent Network, as well as other local organisations.
Main responsibilities
• To maintain contact with patients identified by the Primary Care Networks (PCNs), and associated GP practices.
• To work alongside specific GP practices allocated by the team manager.
• Reach people in the community through different approaches, including community hubs, GP surgeries and on some occasions in people’s homes, as well as making use of digital and telephone support.
• Work with the team manager to ensure that you are clinically competent to undertake all tests required as part of the service.
• Undertake the NHSE physical health checks in line with NHSE best practice guidelines.
• Provide an asset-based, personalised approach in supporting people to better manage their mental health and wellbeing, working with them based on their priorities and needs
• Co-produce a simple, personalised support plan which is goal focused and reviewed
• Effectively manage a caseload, with a good awareness and understanding of when to refer people back or onto to other professionals/agencies when the person’s needs are beyond the scope of the service
• Identify, assess, and effectively manage risk and safeguarding issues.
• Work independently in the community, proactively building up a network of professional relationships to ensure that you are able to support people with a variety of needs.
• Maintain timely, accurate and up to date records relating to individuals and assist in the compilation of quarterly progress reports and other reports as required
• Work flexibly around client referrals and sign ups, adapting to the needs of the team and service when required General
• Undertake and participate constructively in induction, clinical supervision, one-to-ones, appraisal and relevant training, and contribute positively to good team relationships and continuous improvement of services
• Represent the company professionally at external meetings and forums in the local area, maintaining positive relationships with key stakeholders
• Maintain confidentiality in line with organisational policy in relation to those using our services, staff and business sensitive information.
• Any other requirements to ensure effective delivery of the service, within reasonable expectations
